Within the project «Expanding access to home care and palliative care services in 7 Ukrainian communities by a coalition of CSOs — social-service providers», case managers help people who need social support, care and accompaniment to access the services they require.
Case management makes it possible to fully assess a person's needs, identify the help required, organise cooperation with relevant services and organisations, and support the person until their situation is resolved.
452 cases — 450 people who received help
Over the project's eight months of work, case managers have handled 452 cases, of which:
- 324 cases are already closed;
- 128 cases remain active and are still being worked on;
- 450 people have received help through case management.
Case managers work in six oblasts of Ukraine: Kharkiv, Chernivtsi, Poltava, Vinnytsia, Chernihiv, and Volyn. They help those who need support, care, and access to social services.
Behind the case statistics stand specific people and their life stories. These are older adults, persons with disabilities, internally displaced persons, people with severe and incurable illnesses, and families who cannot cope on their own with the need for care and support. A case manager's task is to see the specific need behind every situation and to help find a real solution.

A case manager helps a person avoid being left alone with the system. They identify needs, help find available services and resources, coordinate interaction between different institutions, and support the person while they access assistance.
This approach is an important part of creating an accessible and person-centred system of home care and palliative care.
452 cases are 452 stories in which specialists helped people find solutions, receive assistance, and use the services they needed.
Who implements the project
Case management in the communities is taking place within the project «Expanding access to home care and palliative care services in 7 Ukrainian communities by a coalition of CSOs — social-service providers», the lead implementer of which is NGO «100% Life Rivne Network».
Six member organisations of the Coalition — with many years of experience working with older people, persons with disabilities, children, internally displaced persons, and other vulnerable groups — are involved in the project.

- NGO «Association of Women of Ukraine» — Ternopil;
- AUCO «Parostok» — Vinnytsia;
- NGO «Kiddom» — Shyshaky community and Pyriatyn, Poltava Oblast;
- NGO «Community Development Center “Idea. Action. Result”» — Vyzhnytsia, Chernivtsi Oblast.
The joint work of these organisations is aimed at improving people's access to social services, developing case management, studying community needs, and advocating for systemic changes in home care and palliative care.
